Thesis Completion Verification Statement and Originality Report Upload System

流程：請先下載比對聲明書 OTD(word檔) 或 PDF檔案 填妥並交付指導教授簽名後，再一併將簽署完成的比對聲明書及原創性比對報告一併上傳即可。

Procedure: Please first download the Comparison Statement OTD (Word file) or PDF file, complete it, and have it signed by your advisor. Then, upload the signed Comparison Statement along with the Originality Comparison Report together.

*聲明書 / Declaration Form:

1. 請上傳PDF檔，檔名用「聲明書-學號姓名」 Please upload a PDF file with the filename "Declaration Form-Student ID Name."

2. 簽名方式「可採用：電子簽名、印出後簽名拍照上傳」 Signature options: "You may use either an electronic signature or print it out, sign it, and upload a photo."

申請過程：Application Process:

申請 「論文計畫審查考試」或「學位考試」 時，需提交：
When applying for the "Thesis Proposal Review Exam" or the "Degree Examination", the following documents must be submitted:
- 符合規定的 「第一頁論文資訊」 與 「最後百分比比對報告」。
  The "First Page with Thesis Information" and "Final Page with Percentage Comparison Report" meeting the institute's requirements.
- 經研究生與指導教授簽章確認的 「研究生學位論文完成比對聲明書」。
  The "Graduate Student Thesis Completion Declaration Form" signed and confirmed by the graduate student and their advisor.
請先將上述文件上傳至本所網站後，才能提出「論文計畫審查考試」或 「學位考試」申請。Please upload the aforementioned documents to the institute's website before submitting your application for the "Thesis Proposal Review Exam" or the "Degree Examination."

請上傳：Please Upload:

原創性比對報告的「第一頁論文資訊」 與 「最後百分比比對報告」PDF檔。
A PDF file of the Originality Comparison Report, including only the "First Page with Thesis Information" and the "Final Page with Percentage Comparison Report."
檔名格式：「比對報告-學號姓名」File naming format: "Comparison Report-StudentIDName"

注意事項：Important Notes:

所有研究生需完成論文原創性比對，結果需低於 25%（不包含參考文獻）。All graduate students must complete the originality comparison for their thesis, with a result below 25% (excluding references).
若指導教授另有更嚴格規定，請遵從其規定。If the advisor has stricter requirements, please adhere to their guidelines.
