流程：請先下載著作審查申請書OTD或PDF檔案填妥並交付指導教授簽名後，再一併將簽署完成的申請書及佐證文件(例如：發表證明、期刊封面（含目錄+著作第一頁）一併上傳即可。

Procedure: Please download the Thesis Review Application form in either OTD or PDF format, complete it, and submit it to your advisor for signature. Afterward, upload the signed and completed application form along with supporting documents, such as publication proof, journal cover (including table of contents and the first page of the thesis), all together.
