流程：請先下載 著作審查申請書 OTD 或 PDF 檔案，填妥並交付指導教授簽名後，再將已簽署的 申請書 及 佐證文件（例如：發表證明、期刊封面〈含目錄＋著作第一頁〉） 一併上傳即可。

Procedure: Please download the Thesis Review Application form (OTD or PDF), complete it, and obtain your advisor’s signature. Then upload the signed application together with supporting documents (e.g., publication proof, journal cover including table of contents and the first page of the paper) at the same time.
